Forward membership includes access to in-person doctor visits at any Forward location as well as fully immersive virtual visits. You will also have 24/7 access to Forward's Care Team through the Forward App.

Forward provides the following primary care services to its members:
General Health Assessments: Forward clinicians perform routine check-ups and health assessments to evaluate overall health status, measure vital signs, and screen for potential health risks or conditions.
Preventive Care: Forward clinicians focus on preventive measures to maintain and improve health. This includes immunizations, vaccinations, screenings (such as blood pressure, cholesterol, cancer screenings), counseling on lifestyle modifications (diet, exercise, smoking cessation), and health education to promote healthy behaviors and minimize health risks.
Diagnosis and Treatment of Common Illnesses: Forward clinicians diagnose and treat a wide range of common acute illnesses and minor injuries, including respiratory infections, ear and sinus infections, urinary tract infections, allergies, minor sprains or strains, and skin conditions like rashes or dermatitis.
Chronic Disease Management: Forward clinicians help manage chronic conditions such as diabetes, hypertension, asthma, heart disease, arthritis, or thyroid disorders. The focus is on ongoing monitoring, medication management, lifestyle modifications, and patient education to optimize health outcomes and prevent complications.
Routine Care and Health Maintenance: Forward clinicians provide routine healthcare services including physical exams, gender appropriate care, contraceptive counseling, reproductive health services, and guidance on family planning.
Mental Health Services: Forward clinicians help address common mental health concerns such as anxiety, depression, stress management, and behavioral issues.
Coordination of Care: Forward’s care team helps to manage referrals, coordinate diagnostic tests, review results, and facilitate communication between different providers.
Follow-up Care: Forward clinicians and care team help with follow up care including monitoring, follow-up visits, and management of chronic conditions, ensuring treatment plans are effective and adjusted as needed. Providers help track progress, order necessary labs or imaging studies, and assess the response to treatment.
Forward provides, but does not cover the cost of pap-smears in California. Patients are billed directly by the lab or their insurance for this test. Forward provides and covers the cost of point-of-care lab testing for the following lab tests: Comprehensive Metabolic Panel, Lipid Panel, Rapid Urine Pregnancy Test, Rapid Strep A Test, Rapid Mono Test, Rapid HIV Test, POC Urinalysis, and POC Glucose. In California, Forward dispenses first-fills of the following medications:
Albuterol sulfate
Amlodipine besylate
Ceftriaxone
Doxycycline monohydrate
Triamcinolone acetonide
Injectable Antibiotic Bicillin
Nebulizer Albuterol Solution
Nebulizer Ipratropium Albuterol Solution
Forward locations outside of California may dispense first-fills of other medications, subject to availability and state law.
We're a primary care provider with staff physicians trained in family or internal medicine, so our offerings are geared toward addressing many of your day-to-day needs, while also helping you avoid issues, identify problems early, and control chronic conditions before they become more expensive to treat. Membership also includes all consultations you have with Forward – at our locations, virtually, and in the app.
Services that are not part of our stated offerings are not included in your Forward membership.* These are things like:
Imaging (MRIs, x-rays, CAT scans)
Services requiring referral to a specialist (Cardiologist, Allergist)
Elective or planned hospitalizations
Urgent and emergency care, including hospitalizations
Prescriptions for ongoing needs and those not included in membership (that is, other than common generics kept in-clinic)
Ongoing treatment for severe depression, anxiety, substance use, and other mental health needs
Lab testing not included in membership (that is, other than common tests performed in-clinic)
If you live in California, your Subscription also does not cover:
More than thirty-six (36) in person or synchronous audio-visual telehealth visits with a licensed Forward healthcare provider (e.g., physician or nurse practitioner) during each twelve-month period of your Subscription; and
More than thirty-six (36) in person or synchronous audio-visual telehealth encounters with an unlicensed Forward medical assistant during each twelve-month period of your Subscription (collectively, “California Excluded Services”, and together with Excluded Items, the “Excluded Services”).

If a member needs additional tests or procedures that are not included in their Forward membership, then we will work to help the member try to find a non-Forward option that fits their needs. If the member has insurance, that typically means that we coordinate with their insurance provider to facilitate specialist referrals, prescriptions, or emergency care within their provider network.
Our membership model can help bring predictability to spending on primary care. It is, however, very important to understand that a Forward membership is not health insurance and is not a substitute for comprehensive health insurance. We encourage any person (including our members) who can afford it to maintain a separate comprehensive health insurance plan of their choosing.
Meaningful, lasting changes in health and lifestyle take time. Because of that, your Forward membership begins with a six-month commitment. We believe this gives you, your doctor, and your Care Team the necessary time together to build a lasting relationship and provide you with the support you need to reach your health goals.
You may cancel your membership at any time after the six month period, but we are unable to refund you for payments made before the cancellation.
Refunds take 5-7 business days to be processed and reflected in your account. You're welcome to restart your membership at any time.

You will be charged for your first month of Forward on the day you sign up for your membership. You will then be charged monthly for your membership, on a go forward basis, on the same day each subsequent month. The monthly fee includes all services we provide. See what isn’t included in my Forward membership. Forward will never charge you a copay or send you an unexpected bill for a service we perform.
Individual memberships are $149/month for all our sites.

We only see members over the age of 18.
Yes! You can sign up for the membership using your FSA or HSA card as you would any other credit card.
If you’d like to use your FSA or HSA card to pay for a portion of your purchase and apply the remaining balance to a different card, please reach out via chat or call us at 833-33-GOFWD. We’re happy to help set that up.

We offer certain generic medications onsite at Forward locations. First fills of medications for health conditions such as high blood pressure, high cholesterol, diabetes, asthma, allergies, flu, and issues requiring antibiotics (including z-packs) are included in the Forward membership.
Any prescriptions for medications not carried onsite or for recurring needs (beyond the first fill) are not included in your Forward membership. In these cases, you will be responsible for their cost. If you have insurance, this usually means having the prescription coordinated and billed through your insurance, especially when prior authorizations are required.
We’ll send the prescription to your local pharmacy, and can arrange for you to pick them up or for them to be delivered to you at home!
The Care Team will always let you know if they recommend services not included in the membership and will work with you to anticipate and decrease out-of-pocket costs in case you need additional services (e.g., specialists, hospitalizations, prescriptions, labs, imaging).
